About COMLEX Level 3
COMLEX-USA Level 3 is the final licensing exam required for DOs, typically taken during intern year. Passing Level 3 confirms your ability to independently apply osteopathic medical knowledge and clinical judgment in real-world patient care. It’s a key step toward full, unrestricted licensure and unsupervised practice.
Level 3 is a two-day, computer-based exam administered over four testing sessions within a 14-day period. Each day consists of two 3.5-hour sessions, totaling approximately 9 hours of testing per day, including break time and tutorial.
The exam includes:
• 420 multiple-choice questions spread across four sessions
• 26 Clinical Decision-Making (CDM) cases, which test your ability to gather, interpret, and apply patient data in time-sensitive scenarios
Day 1 focuses on foundational clinical knowledge and includes four sessions of 70 multiple-choice questions each (280 total).
Day 2 features one session with 13 CDM cases followed by another session with 140 multiple-choice questions.
The CDM format is unique to COMLEX and emphasizes real-time clinical reasoning. These cases typically include 2–4 short-answer questions that simulate real-world decision-making, such as ordering tests, choosing treatments, or determining follow-up.
To pass COMLEX Level 3, you must score at least 350. The national average is approximately 520, with a standard deviation around 85. First-time pass rates are high—often exceeding 96%—but the exam still demands deep clinical understanding and strong test-taking skills, especially with the addition of CDM.
