What Is the Casper Exam?
Used by medical schools to evaluate applicants, the Casper test is a crucial element of the medical school application process. This online test, administered by Acuity Insights (formerly known as Altus Suite), is intended to assess key personal and professional characteristics that are difficult to capture through the other aspects of your medical school application.
Test Format and Content
The Casper test, now a component of many med school applications, takes 65-85 minutes to complete and comprises 2 distinct sections.
The first is the video portion of the test, which includes four different scenarios—a mix of video-based and word-based. Instead of typing your responses, you will have one minute to record your verbal response to two questions, with one minute per question.
Next comes the typed response section, which requires you to type your answers to two open-ended questions asked after viewing or reading seven different scenarios, a mix of video-based and word-based.
Casper Scores and Evaluation
Unlike traditional exams, the Casper test does not assign scores based on right or wrong answers. Instead, your responses are scored relative to those of other test takers by trained raters from various professional backgrounds. Each response is rated on a scale from 1 (unsatisfactory) to 9 (excellent), with no single rater scoring more than one of your responses to prevent bias.
Results are typically released within 2-3 weeks after the test, and Acuity Insights sends them directly to the medical schools selected on your distribution list. While you won’t receive individual scores for your responses, you will learn which quartile your overall performance falls into, giving you a broad sense of how well you performed compared to your peers.
This is how the quartiles break down:
- 25% of applicants score in the first quartile (0-24 percentile)
- 25% of applicants score in the second quartile (25-49 percentile)
- 25% of applicants score in the third quartile (50-74 percentile)
- 25% of applicants score in the fourth quartile (75-100 percentile)
If you land in the fourth quartile, it means the raters judged your responses as superior to 75% of your peers. If you land in the first quartile, it means the raters felt 75% of your peers provided answers that were more comprehensive and empathetic than yours, which is obviously not a good sign to medical school admissions committees.
